I (We), _______________________, have received and read the Cindi Bear Daycare Policies and Procedures and will comply with all provisions contained therein, and shall at this time enter into an agreement with Cindi Manning, of Cindi Bear Daycare, for the care of my (our) children:

Friday	a.m.	p.m.

Lack of enforcement of a certain policy at any time does not indicate that the particular policy is no longer in effect.
